Financial

Payment policy
It is our policy to collect the appropriate payment due from the patient at the time of service. This may only be your copayment, deductible and/or coinsurance, but we do ask for payment at the time of your visit. We accept most major credit cards. Always bring your insurance card for every visit.

  Co-payment is the cost sharing part of your bill. This is a fixed dollar amount designated by your insurance company. This amount is your responsibility to pay at each visit. (Also known as “co-pay”). Common copayment rates are 10.00-30.00 per visit, but be aware that co-pay rates vary from insurance companies.
  Deductible The part of your bill, in addition to co pays which you must pay for medical services before your health insurance starts to pay, usually the first 3 months of the year.
  Coinsurance - the part of your bill, in addition to co pay, which you must pay. Coinsurance is usually a percentage of the total medical bill- for example, 20%.

We request that accounts be paid in a timely manner and kept up to date. If you have questions after reading this information, we will be happy to answer them for you.

Office

Children of divorced or separated parents
Payment for services is required by whichever parent brings the child into the office for services. Divorce agreements are between the two parents and do not affect our office. We will provide you with a paid receipt for these services.

Appointments:
We request that you call for all appointments. It is important that you keep schedules appointments. If you are a new patient, please arrive 15 minutes before your scheduled appointment to complete registration forms. Failure to cancel appointments can disrupt the office flow and prevent other patients from being seen. Three missed appointments within one year (without canceling), will result in dismissal from the practice. Please give us 24 hour notice if you need to cancel your appointment. We will strive to meet your medical needs within the allotted time, but please know that emergencies can happen and could cause a delay in your appointment time. We are required to triage patients at times, which may affect the schedule. Please be prepared to wait if necessary.

Medical Records
Our medical records department is open from 8:00-2:00 Monday-Wednesday. Please try to provide our office with your previous records prior to your first visit so the physician can review.

To have your records released from this office, you will need to complete and sign a request form indicating the physician to whom the records should go.

Medication Refills
Medication refills should be called in to us one week before the prescription is finished. When you call, please give your name, date of birth, phone number, and name of the drug, prescription number and pharmacy. For long distance pharmacies, you will need to provide us with a toll free fax number or have the pharmacy call us for your refill. If your prescription needs to be written, please indicate the quantity and number of refills you need when you call the office.

We will make every effort for your medication to be called in by 5:00 pm daily. Please call your pharmacy before you go.

It is important that you check your prescriptions and not run out of medication. Please check your medication to be certain you will not be out when the office is closed or the physician is out of town.

Narcotics and anti-anxiety medicines will only be refilled by the physician who prescribed them and only during office hours when the patient’s chart is available for reference. Medications not prescribed by Dr. Bowlin will not be refilled by this office.

Disability Claims
Your doctor is responsible for deciding if disability, FMLA, and time off work forms are appropriate for you. They will be completed in a timely manner and there will be a 15.00 charge for each form completed by your physician. Please know FMLA forms will not be completed for routine illnesses. FMLA is intended for chronic illnesses and will be treated as such.

Work/School Excuses
All patients must ask the physician for a work/school excuse while he/she is still in the exam room. The receptionist will then write out your release and make a copy for your chart.

Hospitalists
If you must be admitted to the hospital, you will be cared for by hospitalists. Hospitalists are a group of physicians who specialize in hospital care. Diagnosis and treatment will be provided and you will be given the best possible care while in the hospital. Upon discharge, you will be instructed to follow up with Dr. Bowlin, who will resume your care.

Minor Children
All minor children must be accompanied by a parent or consented adult with every visit. Never leave your child unattended in any area of the office.
